We are experts at working  with those living with Alzheimer's, Geriatrics, Diabetes, Cognitive Impairment, Disabilities or anyone interested in having a Health Care Advocate on their side.

Our services are designed for older adults taking multiple medications (polypharmacy), those with chronic conditions like dementia, Alzheimer's, or delirium, those recently discharged from the hospital, and anyone experiencing medication-related side effects or challenges. We also provide invaluable support and guidance to family caregivers who are managing their loved one's medications. 

A geriatric pharmacist specializes in medication for older adults. They understand how aging affects medication responses, drug interactions, and managing multiple conditions. 

We're a consulting service, not a retail pharmacy. We focus on in-depth medication reviews, personalized plans, and working with your doctors to optimize your loved one's medication regimen. 

Grandparents Pharmacist is a private-pay, subscription based service. We do not bill insurance directly. 

Ongoing support, regular medication reviews, personalized consultations, proactive monitoring, and adjustments as needed. Think of it as having a dedicated geriatric pharmacist on your team. 

We have monthly, 3-month, and 6-month plans. All include core services like medication reviews and consultations, with longer terms offering greater value.

Yes, cancel anytime. Access continues until the end of the billing cycle. No refunds are provided. 

We do not offer refunds for our subscription.

Our pharmacist reviews all medications, over-the-counter drugs, and supplements. We assess for interactions, effectiveness, proper dosages, and discuss any concerns you may have.

Gather all medications and supplements. Have a list of all medical conditions. Write down your concerns and questions. Any relevant medical records handy are helpful, however we can help pick the most relevant ones to share.

 Initial consultations are about 60-90 minutes. Follow-ups may be shorter. However, we will take as long as is reasonably needed to hear all of your concerns and to address them to your satisfaction.

We primarily offer virtual consultations. In-home visits may be possible in some locations within the Portland Metro area.

You can reschedule your appointment by contacting us through our HIPAA-compliant email, info@grandparentspharmacist.com. Please contact us as soon as you know you will need to reschedule. Cancellations may result in delayed rescheduling due to limited availability.

No, we're a consulting service and cannot prescribe. However, we work closely with your loved one's doctor to suggest medication changes or deprescribing if needed. 

No, we are not a dispensing pharmacy. You'll continue to use your regular pharmacy. 

We'll get to know you and your loved one, discuss your concerns, explain our services, and recommend a plan. There's no obligation to subscribe. 

Polypharmacy is taking multiple medications, which is common in seniors. It increases the risk of interactions and side effects. We specialize in managing polypharmacy. 

 Deprescribing is safely reducing or stopping medications that may no longer be beneficial or could be causing harm. We can guide this process with your doctor.
